INCIDENT REPORT

Unauthorized Device Behavior and Security Concern – iPhone 17 Camera & eSIM PIN

Reporting Party: Sarai Hannah Ajai

Device Owner: Same as above

Device: Apple iPhone 17

Carrier: V****** Wireless

SIM Type: eSIM with 4-digit SIM PIN enabled

Date Device Received: October 22, 2025

1. Date and Time of Incident

December 10, 2025, at approximately 3:45 PM (local time)

2. Location

Private residence (kitchen area).

3. Description of Incident

On December 10, 2025, at approximately 3:45 PM, I was preparing to take a photograph of my kitchen using the rear-facing camera on my iPhone 17. The device was in my physical possession at all times.

As I opened the Camera application and was about to press the shutter button to take the picture of my kitchen, the camera view suddenly switched from the rear-facing camera to the front-facing (selfie) camera without any intentional action on my part to change the camera orientation.

I did not manually tap the screen to switch cameras, did not activate any on-screen icon to flip the camera, and did not use any known gesture, button, or voice command that would normally trigger a change from the rear camera to the front camera.

This unexpected and unintentional camera flip created an immediate concern that the device’s camera behavior might be affected by factors outside my intended use or control.

4. Security Measures in Place at the Time of the Incident

At the time of the incident, the following security measures and conditions were in effect:

The iPhone 17 is a recently purchased device, received on October 22, 2025.

The device is protected by Apple’s standard iOS security controls.

The device Screen Time is activated with PIN passcode.

The device uses a V****** Wireless eSIM.

A 4-digit eSIM PIN is enabled for the V****** line.

The device and SIM/eSIM have not been intentionally shared with or provided to any third party.

The device was physically in my possession and located inside my residence at the time of the incident.

5. Basis for Concern

My concern arises from the combination of:

An unsolicited and unexplained change in the camera orientation at the precise moment I was about to take a photo.

The fact that my V****** eSIM is protected by a 4-digit SIM PIN, which is intended to prevent unauthorized access or use of the line.

Prior concerns regarding potential unauthorized access, mirroring, or cloning of my devices.

This leads me to question whether:

There is an iOS or app-related malfunction or glitch;

There is an input or gesture misinterpretation within the camera application; or

There is any possibility of unauthorized remote access, device mirroring, or device cloning occurring despite the presence of a 4-digit eSIM PIN.

At this time, I am not making a definitive technical conclusion. I am documenting the behavior, my security configuration, and my related concerns for expert review.

6. Specific eSIM PIN and Mirroring/Cloning Question

Because my V****** eSIM is protected by a 4-digit PIN, I have a specific technical and legal concern:

If any third party, such as a hacker or unauthorized individual were able to gain control of, mirror, or clone my iPhone 17 or otherwise operate its functions remotely:

1. Would they first be required to correctly enter my V****** eSIM 4-digit PIN in order to:

o Use the cellular service;

o Access or duplicate the line; or

o Mirror or clone the device credentials associated with that eSIM?

2. Under what technical conditions, if any, could a person or system gain access to or control over the device’s functions (including the camera) without having entered the correct V****** eSIM 4-digit PIN?

3. How, if at all, could an attacker bypass or override the V****** eSIM 4-digit PIN protection to mirror, clone, or remotely control a device that appears to be secured by that PIN?

These questions are being documented for the purpose of:

Requesting a clear, written explanation from Apple and V****** regarding how eSIM PIN security interacts with potential device mirroring, cloning, or remote access; and

Understanding whether the behavior I observed could be consistent with normal software behavior or might indicate a security vulnerability or compromise.

7. Absence of Lawful Process or Warrant

I also note the following:

To the best of my knowledge and recollection, no law-enforcement agency or other authorized entity has ever served or presented a search warrant, court order, or valid legal process directed at me or my device in connection with this incident or any related matter.

At no time have I been notified of any legal request or been asked to produce evidence, nor have I signed any consent form related to access of my iPhone 17, eSIM line, or camera functions.

Because of this absence of legal process, I find it particularly important to record a complete account of this incident as it occurred, including the security measures in place and my outstanding technical questions.

8. Actions Taken

Immediately noted the time, date, and circumstances of the camera behavior.

Ceased nonessential use of the device to avoid further potential exposure until I can receive guidance.

Prepared this written incident report for record-keeping and possible submission to:

o Apple Support and/or Apple Security;

o V****** Wireless Technical Support or Fraud/Security Department;

o Legal counsel; and/or

o Appropriate authorities, regulators, or property management, if warranted by further findings.

9. Statement of Accuracy

I affirm that the information contained in this report is true and correct to the best of my knowledge and recollection, and that it is intended to document observable facts, existing security settings, and reasonable technical questions not to assert a specific technical conclusion in the absence of expert analysis.

Respectfully,
Sarai Hannah Ajai
