INCIDENT REPORT
Repeated Apple iPhone 17 iCloud Two-Factor Authentication Security Key Failure (Fourth Re-Registration Event)
Reporting Party: Sarai Hannah Ajai
Device: Apple iPhone 17
Associated Account: Apple iCloud (Sign-In & Security → Two-Factor Authentication → Security Keys)
Carrier: V*r*z*on W*r*l*s*
Location: Fargo, North Dakota
Date of Incident: February 18, 2026
Approximate Time: Between 5:40 PM and 6:10 PM (local)
I. SUMMARY OF INCIDENT
On February 18, 2026, I was required to re-register my Y*b*co physical security keys for the fourth time since February 15, 2026 in order to restore functional access to my Apple iCloud account under Sign-In & Security → Two-Factor Authentication → Security Keys.
This action was necessary after my previously registered security keys again failed during the Apple two-factor authentication process, resulting in repeated error messages stating “Failed to verify your identity. Try again.” despite the keys being physically present, properly configured, and protected by valid PINs causing me to experience identity thefts.
Screenshots documenting the re-registration process, security-key removal prompt, and subsequent successful browser trust verification are attached as exhibits.
II. PRIOR HISTORY AND PATTERN
Since February 15, 2026, I have experienced a recurring pattern of:
• Security keys unexpectedly failing Apple iCloud authentication
• Forced removal and re-registration of physical security keys
• Temporary account access disruption
• Reversion to less secure authentication methods during recovery
This represents the fourth re-registration event in three days, which is highly irregular for hardware-based authentication mechanisms designed to be resistant to tampering.
III. SAFETY-RELATED CONTEXT (DOCUMENTED, NOT CONCLUSIONS)
On or around the same timeframe as the most recent authentication failure, a neighboring tenant (Apartment Unit **6) made verbal statements to me claiming involvement in hacking my Apple iPhone 17. These statements are reported here solely as statements made, not as verified facts or conclusions.
I am documenting this interaction because of:
• The temporal proximity to repeated account security disruptions
• My ongoing concerns regarding personal safety, privacy, and device integrity
• The cumulative stress caused by repeated unauthorized-appearing account failures
No technical determination has been made by me regarding the validity of those claims.
IV. IMPACT
As a result of this incident and the ongoing pattern:
• My Apple iCloud account integrity has been repeatedly compromised
• I have lost confidence in the stability of my device’s security posture
• I have been forced to repeatedly reset security infrastructure designed to prevent unauthorized access
• My sense of personal safety and digital privacy has been materially affected
V. EVIDENCE ATTACHED
• Exhibit A: Screenshots showing security-key removal prompt
• Exhibit B: Screenshots showing re-registration of Y*b*co security keys (fourth instance)
• Exhibit C1 and C2: Screenshots showing Apple iCloud authentication error messages
• Exhibit D: Screenshots showing restored access after re-registration
VI. PURPOSE OF REPORT
This report is submitted to:
• Preserve a contemporaneous record of events
• Document a recurring security anomaly affecting my Apple iCloud account
• Support any future technical, administrative, or legal review
• Formally express concerns regarding personal safety and digital security
I respectfully request that this report be treated as a good-faith documentation of observable events and impacts, without presumption or speculation.
