 Suspected Identity Theft, Unauthorized Signature Forgery, Misrepresentation of Legal Authority, and Potential Unauthorized Access to Personal and Business Property

Date of Report: October 21, 2026
Complainant: Sarai Hannah Ajai
Complaint Classification: Suspected Identity Theft, Identity Misrepresentation, Potential Document Forgery, and Unauthorized Account Access
Reporting Status: Supplemental Consumer Identity Protection Complaint

I. FORMAL STATEMENT OF COMPLAINT

I, Sarai Hannah Ajai, submit this Incident Report to formally document my concerns regarding suspected identity theft, possible signature forgery, misrepresentation of personal or legal authority, and recurring security problems involving my personal and business electronic accounts.

I have experienced repeated difficulties involving my Apple devices, Apple Account credentials, Verizon wireless account, Screen Time passcodes, and other authentication-related information.

These incidents have caused significant concern because I have not knowingly authorized any neighboring resident, property-management employee, acquaintance, family member, alleged romantic partner, business associate, or other unauthorized third party to access, administer, control, or modify my electronic accounts or personal and business property.

I am also concerned that my personal identity or signature may have been used without my knowledge or authorization to create documents falsely representing that another individual possesses legal authority over me or my property.

At the time of this report, I have not independently authenticated a specific forged document establishing these suspected acts. I request appropriate verification of any documents or authorization records that may exist.

II. EXPRESS DENIAL OF UNAUTHORIZED RELATIONSHIP-BASED AUTHORITY

I expressly dispute any representation that an individual possesses credential authority, device-access authority, agency authority, account-management privileges, or property-management rights over my personal or business assets solely because that individual alleges or asserts the existence of any of the following relationships:

  1. Marriage or purported spousal relationship;
  2. Romantic relationship;
  3. Domestic relationship or domestic partnership;
  4. Familial relationship;
  5. Employment relationship;
  6. Housing, tenancy, or residential relationship;
  7. Caregiving relationship;
  8. Business relationship; or
  9. Any other alleged personal, contractual, or professional association.

I have never been married.

I have not knowingly executed a marriage certificate, marriage license, spousal authorization, domestic partnership agreement, or related instrument giving another person authority over my personal identity, credentials, financial accounts, or electronic property.

I further dispute any alleged authorization purportedly arising from a forged signature, falsified consent, impersonation, or inaccurate representation of a relationship.

An asserted relationship, standing alone, must not be treated as evidence that I granted account access, delegated authority, executed a power of attorney, or transferred ownership or management rights.

III. SUSPECTED FORGERY OF SIGNATURES AND LEGAL INSTRUMENTS

I am concerned about the possibility that unauthorized individuals may have prepared, submitted, or relied upon documents falsely bearing my signature, identifying information, or alleged consent.

Potential documents of concern include:

  • Marriage certificates or marriage licenses;
  • Domestic partnership declarations;
  • Powers of attorney;
  • Agency authorization documents;
  • Affidavits or sworn declarations;
  • Consent and authorization forms;
  • Waivers of rights;
  • Telecommunications account authorization forms;
  • Electronic account-access agreements;
  • Financial account authorization instruments;
  • Business ownership or management documents;
  • Intellectual property assignments; and
  • Other instruments purporting to delegate authority or establish obligations without my informed authorization.

I expressly dispute the validity of any document containing a signature attributed to me that I did not execute or authorize.

I request that any identified document be assessed through its original record, submission history, signature information, and applicable authentication requirements.

I understand that the existence of a forged document must be established through evidence and that my current concerns do not independently prove that such documents have been created.

IV. PERSONAL AND BUSINESS PROPERTY SUBJECT TO PROTECTION

My concerns extend to potential unauthorized access, administration, transfer, disclosure, or modification of the following property and information.

A. Apple Devices and Digital Identity

  • Apple iPhone 17;
  • Apple Mac mini M1;
  • Apple Account and associated services;
  • Device sign-in credentials;
  • Device passcodes;
  • Screen Time passcodes;
  • Passwords and passkeys;
  • One-time passcodes (OTP);
  • Multifactor authentication credentials;
  • Authentication and session tokens;
  • Account recovery information; and
  • Associated personal information.

B. Telecommunications and Electronic Communications

  • Verizon wireless account;
  • Assigned telephone number;
  • Subscriber identity information;
  • Account security PINs;
  • Email accounts;
  • Electronic communications;
  • Wireless account-management permissions; and
  • Related telecommunications records.

C. Security and Surveillance Accounts

  • Personal surveillance system accounts;
  • Device administration credentials;
  • Security system access permissions;
  • Associated authentication records; and
  • Related account recovery information.

D. Financial, Business, and Intellectual Property

  • Personal and business financial accounts;
  • Software source-code repositories;
  • GitHub accounts and repositories;
  • Application databases;
  • Domain names and DNS management accounts;
  • Business records;
  • Business authentication credentials;
  • Proprietary software and technical documentation;
  • Intellectual property; and
  • Associated contractual and ownership records.

I have not knowingly granted any unauthorized third party administrative authority or ownership rights over these assets.

V. RECURRING APPLE AND VERIZON SECURITY CONCERNS

I have previously documented recurring problems involving Apple device settings, Screen Time passcode recovery, Verizon account credentials, and other authentication-related activities.

I maintain that I have not voluntarily shared my private credentials or device passcodes with neighboring tenants or family members.

The recurring nature of these incidents has caused me to question whether unauthorized access, inaccurate account authorization records, technical configuration problems, or other security failures may be involved.

I request that relevant account providers review available authentication records, device inventories, credential changes, account-management permissions, recovery events, and customer-service authorization records.

I specifically request verification of whether any person claimed authority to access or manage my accounts on my behalf, and, if so, the evidence and authorization procedures relied upon.

I do not assert that a forged legal document has been established as the cause of these technical incidents.

VI. SUPPLEMENTAL RESIDENTIAL INCIDENTS — OCTOBER 20–21, 2026

On October 20 and October 21, 2026, at approximately 9:50 AM on each date, I heard what appeared to be a female voice shouting from the vicinity of my residential building's foyer.

I recall the words:

"I AM YOUR HUSBAND."

I did not establish the identity of the speaker, whom she was addressing, or whether either statement was directed toward me.

Given my concerns regarding identity misrepresentation and unauthorized assertions of authority, I found the repeated statement distressing.

I include these observations solely as supplemental context. I cannot establish that the statements were connected to identity theft, forged documents, unauthorized account access, or any other unlawful activity.

VII. REQUEST FOR VERIFICATION AND APPROPRIATE REVIEW

I respectfully request that the Incident Report receive and document these concerns and provide applicable identity-theft recovery guidance.

Where supported by evidence and applicable law, I seek assistance with:

  1. Identifying and disputing unauthorized use of my personal identifying information.
  2. Determining whether accounts, transactions, or authorization instruments have been established using inaccurate or fraudulent identity information.
  3. Obtaining relevant records from businesses when legally available.
  4. Disputing unauthorized account-management changes or transactions.
  5. Protecting personal and business credentials from unauthorized use.
  6. Correcting inaccurate records that falsely attribute consent or authority to me.
  7. Identifying appropriate agencies or procedures for any verified document forgery, identity theft, or unauthorized access.

I also request that account providers not treat an asserted marriage, romantic association, familial connection, residential association, or business relationship as sufficient authorization to disclose my private information or grant account administration privileges.

Any claimed delegation of authority should be evaluated against applicable law, provider requirements, and authentic authorization records.

VIII. DECLARATION REGARDING CONSENT AND PROPERTY RIGHTS

For the avoidance of doubt, I affirm the following:

I do not consent to any unauthorized person accessing, managing, transferring, disclosing, modifying, or exercising control over my personal or business property, electronic accounts, credentials, or identifying information.

I dispute any unauthorized representation that I have executed a marriage document, power of attorney, waiver, account delegation, agency appointment, or other instrument granting such authority.

Nothing in this complaint should be interpreted as acknowledging the existence or validity of any disputed relationship or authorization document.

I reserve the right to supplement this report with additional documentary evidence, provider correspondence, authentication records, or verified findings as they become available.

IX. CONCLUSION

I respectfully submit this complaint to document my concerns regarding identity protection, possible unauthorized document execution, recurring account-security incidents, and potential misuse of representations of personal or professional relationships.

My objective is to obtain accurate records, protect my personal and business information, and identify any unauthorized activities that can be established through reliable evidence.

I request that all allegations be evaluated individually and that no finding concerning forgery, identity theft, or unauthorized access be made without adequate supporting documentation.
